Knocking Blood Transfusions Below the Bar [Slides]
The Good Samaritan Hospital (GSH), Cincinnati team elected to collect hospital wide transfusion data specific to service lines, physicians, and DRGs. Elective orthopedic surgeries were three of the top fifteen DRGs highest in the use of packed red blood cells. Among the orthopedic physicians there was a wide range of practice variation with physician specific transfusion rates varying from 5% to 80% for hips and knees. Overall transfusion rates in all elective total joints were 22% to 58%. Specific surgeon data was blinded and presented at the orthopedic section meeting; PEER data was available for comparison. It was at this time the section of orthopedics decided to change practice behaviors. Physician champions were identified and met as a focus group committed to develop strategies based on evidence based best practices. With the use of these strategies, GSH has maintained a transfusion rate of 2-5% in elective total joint arthroplasties and decreased infection rates.
